Both drugs work the same way
Sildenafil and tadalafil work in exactly the same way. They both block an enzyme called PDE5, and both bind to the same site on that enzyme. There is only one mechanism of action between the two drugs, so combining them does not add a second one.
What the combination does add. Both medications work by widening blood vessels, and the two effects together create a greater risk of dangerously low blood pressure. That risk rises further alongside alcohol or any other medicine that lowers blood pressure.
Two very different durations. Sildenafil has a half-life of around four hours. Tadalafil has a half-life of about seventeen and a half hours, and its effect can be felt for far longer. Taking a tablet containing both means the shorter-acting drug has cleared while the longer-acting one is still in the body.
Why this matters in an emergency. If someone needs nitrate treatment for a heart attack, the wait after taking sildenafil is twenty four hours. After tadalafil it is forty eight. A tablet containing both carries the longer window, and anyone treating you needs to know exactly what was taken and when.
Nitrates. Nitrates must never be taken with either drug. That includes glyceryl trinitrate and isosorbide prescribed for angina, and amyl nitrite sold as poppers. The combination can lower blood pressure to a fatal level.

Is it better to take two ED drugs together?
Standard practice is to use one PDE5 inhibitor and adjust its dose rather than to combine two. If a single drug is not working, the useful questions are about timing, alcohol, other medicines and the underlying cause.
Is Sildalist licensed anywhere?
No regulator has licensed a tablet containing both sildenafil and tadalafil. Each drug is licensed separately for erectile dysfunction and both are well established individually. The combination is a manufacturing decision rather than an approved formulation.
What are the side effects?
The side effects of both drugs overlap and can add together. Headache, flushing, indigestion, a blocked nose, dizziness and back pain are all reported. Back and muscle pain is more associated with tadalafil than with sildenafil.
