Why does a 37 mcg patch exist?
The odd number comes from the way the strengths were built. Each dose is a multiple of the smallest one, so the range runs 12, 25, 37, 50, 75 and 100 micrograms an hour. Three of the smallest units come to 37.5, and the label drops the half exactly as it does on the 12 mcg patch.
Moving up a step. Increases are made no faster than every three days, because that is how long a new strength takes to settle. The patch currently on the skin comes off, the new strength goes on at a different site, and the change is judged over the following couple of days rather than the first afternoon.
Side effects at 37 mcg. Nausea, constipation and drowsiness carry on at this strength. A step up will generally bring a few days of feeling sleepier before the effect levels out. Any confusion or slowed breathing after a change in strength should be looked at rather than ignored.

The full range is 12, 25, 37, 50, 75 and 100 micrograms an hour, so 37 represents three of those small units. Rounding the half away on the label is the same thing that happens with the 12 mcg patch.
Can you use two fentanyl patches at the same time?
Only if a prescriber has told you to. Two patches means two release rates added together, and that is a dose decision rather than something to improvise. Where it is done deliberately, both patches go on and come off at the same time.
How long does a new fentanyl transdermal patch strength take to settle?
Give it two to three days. Fentanyl moves slowly through the skin in both directions, so the new strength takes that long to reach a steady level and the old one takes a similar time to fade. Judging a change on the first day gives a misleading picture either way.
