What is the 75 mcg fentanyl patch used for?
Persistent pain in someone already established on opioids and already stepped up through the lower strengths. Nobody arrives at this level quickly, since increases are made no faster than every three days.
Other medicines matter more here. Interactions count for more at this strength than most people expect. Fentanyl is broken down by the liver enzyme CYP3A4. Anything that blocks that enzyme will leave more fentanyl in the blood than is intended. Some antifungals, some antibiotics including erythromycin, certain HIV medicines and the heart drug diltiazem can all do this.
The risk is not theoretical. One of those medicines added to a stable patch dose has resulted in serious breathing problems and in deaths. Always tell any doctor or pharmacist that you wear a fentanyl patch before starting anything new, including medicines bought without a prescription.
Alcohol and sedatives. Alcohol, sleeping tablets and benzodiazepines add to the same effect from a different direction. Each of them slows breathing on its own, and combined with fentanyl the danger is far greater than either part suggests.
Side effects of the 75 mcg fentanyl patch. Drowsiness, constipation and nausea continue to occur at this level. The signs that need emergency help rather than watchful waiting are breathing that slows or turns shallow, sleepiness that cannot be roused, and confusion. Call 112.

Tell them before you start anything new that you wear a fentanyl patch, and at what strength. Grapefruit juice belongs on the same list, since it inhibits the same enzyme that breaks fentanyl down. Painkillers such as paracetamol and ibuprofen are not a problem, but anything sedating needs checking first.
Can you drink alcohol while wearing a fentanyl transdermal patch?
It is not worth the risk. Alcohol and fentanyl both slow breathing, and the effect of the two together is greater than adding them up would suggest. This matters most in the hours after a patch change, when blood levels are rising.
Why is 75 mcg not simply three 25 mcg patches?
It can be, where a prescriber has decided on it, but a single patch is preferred when one exists at the right strength. Fewer patches means fewer edges to lift, fewer sites to track and less chance of one being forgotten on the skin.
