What is a fentanyl 200 mcg zuigtablet used for?
Breakthrough cancer pain, and only that. This is for people who are already on a regular around-the-clock opioid but who get flares of pain through the background cover. It is not a general painkiller, it is not for pain after an operation, and it is not for anyone whose body is not already used to opioids.
Why everyone starts at 200 micrograms. Everyone starts at 200 micrograms, whatever dose of background opioid they happen to be on. People are surprised at this, since they expect a larger regular dose to earn a bigger starting lozenge. It does not work that way. The right strength is found by starting at 200 and working upwards over successive episodes, never by calculating from the patch or tablets already in use.
How to use it. Place it between the cheek and the lower gum and move it around against the inside of the cheek. Suck it, do not chew it, and do not swallow it whole. Chewing sends the dose down into the stomach, where it works slowly and unpredictably instead of through the cheek. One unit should take about fifteen minutes to finish.
Side effects of the fentanyl zuigtablet. Drowsiness, nausea, constipation and a dry mouth are the common ones. Because it acts quickly, drowsiness tends to set in within minutes rather than building slowly. Breathing that slows or turns shallow, or sleepiness you cannot rouse someone from, needs emergency help. Call 112.

Is a fentanyl buccal tablet the same as a zuigtablet?
They are not the same product, but close relatives. A zuigtablet is a lozenge that you suck against the cheek, whereas a buccal tablet dissolves in place between the gum and the cheek. As the rate of absorption is different, the strengths of these products are not equivalent, and one cannot be used as a replacement for the other without a prescriber recalculating.
How many fentanyl lozenges can you take in a day?
Four episodes a day is the ceiling. If you find yourself needing more than that, it is a signal that the background pain relief is not holding. That is a conversation with your prescriber rather than a reason to take more lozenges.
Is 200 mcg of fentanyl in a lozenge the same as 200 mcg from a patch?
No, and this is the most important thing to understand about it. Fentanyl products are not interchangeable microgram for microgram. Two hundred micrograms in a lozenge is not 200 micrograms an hour from a patch, and it is not 200 micrograms of another oromucosal product either. They are absorbed differently, so each is dosed on its own terms, and switching between them is always a prescriber's calculation.
