What is modafinil used for?
Narcolepsy occurs when the brain cannot regulate sleep and waking properly, causing people with the disorder to fall asleep during the day regardless of how much sleep they get at night. Modafinil reduces that daytime sleepiness. It does not cure narcolepsy, and it does not replace the sleep lost to the disorder.
Why the European licence covers only narcolepsy. In January 2011 the European Medicines Agency reviewed modafinil and concluded that the benefits only outweighed the risks for patients with narcolepsy. The Agency removed three uses from the licence at that point: idiopathic hypersomnia, sleepiness caused by obstructive sleep apnoea, and chronic shift work sleep disorder.
Two problems drove that decision. One was the link to serious, sometimes life-threatening skin reactions, with the risk appearing higher in children. The other was neuropsychiatric effects. For every use other than narcolepsy, those risks were judged to outweigh the evidence of benefit.
How it is taken. The usual dose is 200mg per day. It can be taken as a single dose in the morning, or split into two doses, one in the morning and one at midday. Taking it late in the day interferes with sleep that night, which is counterproductive in a condition that already disrupts sleep.
Who should not take it. Modafinil is not for anyone under eighteen. It is not for women who are pregnant or breastfeeding. It is not suitable for people with uncontrolled high blood pressure or with an irregular heart rhythm. It also reduces the effectiveness of hormonal contraception, so an additional or non-hormonal method is needed.
Side effects of modafinil 200mg. The common side effects are headache, nausea, anxiety and difficulty sleeping. Any rash needs medical attention rather than watching, because of the skin reactions that led to the licence being narrowed. Chest pain, a racing heart or changes in mood should also be reported.

What is the difference between modafinil and armodafinil?
Modafinil contains a mixture of the two mirror-image forms of the molecule. Armodafinil contains only one of those forms. The terminal half-life of the two is similar, but armodafinil holds a higher blood level later in the day. Their licences also differ: modafinil is authorised in Europe, while armodafinil is not separately authorised there.
Is modafinil a smart drug?
It is often described that way, but it is not a licensed use anywhere. Modafinil is approved for a sleep disorder, not for improving concentration in people who sleep normally.
