Where the 1mg dose sits
Treatment with Ozempic starts at a dose of 0.25mg once a week for four weeks. This initial dose allows the body to adjust and is not intended to control blood sugar. The dose then increases to 0.5mg and then to 1mg. The dose of 1mg is a maintenance dose rather than a step on the way to one, and 2mg is available for those who require more effective control of blood sugar.
The pen. Ozempic is supplied in pens with a dial to set the dose. A new needle is used each time the medication is injected. The pen in use can be stored at room temperature for a period of time. Pens that have not yet been opened should be stored in the fridge and should never be frozen. Any pen that has been frozen should not be used.
Injection sites. The injection is administered under the skin of the abdomen, thigh or upper arm. The injection site should be changed each week rather than using the same spot every time. Repeated injections in the same area can cause that tissue to thicken, and thicker tissue results in less reliable absorption of the medication.
Side effects. Nausea, vomiting, diarrhoea, constipation and reduced appetite are the common effects, especially following an increase in the dose. Severe abdominal pain radiating into the back could be a sign of a problem requiring urgent assessment. People with a personal or family history of medullary thyroid carcinoma, or of multiple endocrine neoplasia type 2, should not take semaglutide.

How does Ozempic work?
Semaglutide mimics a hormone called GLP-1, which is naturally released by the gut after eating. Semaglutide causes the pancreas to release insulin when blood sugar levels are high and reduces the release of glucagon, the hormone that raises blood sugar levels. Semaglutide also slows the emptying of the stomach and acts upon appetite. Because the insulin releasing effects of semaglutide are reliant upon blood sugar levels being raised, semaglutide alone is unlikely to cause low blood sugar levels.
Is Ozempic the same as Wegovy?
Ozempic and Wegovy both contain semaglutide but are licensed for different purposes and at different doses. Ozempic is licensed in Europe for type 2 diabetes and is not licensed for weight loss. Wegovy contains the same active ingredient at 2.4mg once a week and is licensed for the treatment of obesity, and for overweight individuals with a weight-related condition. Despite the licence for Ozempic not including weight loss, it is widely prescribed for that reason. Where that happens it is prescribing outside the licence, which is a decision for a doctor to make and to explain.
Ozempic versus Mounjaro: which is better?
Neither of the medicines is clearly the better of the two, because each has a genuine advantage. Ozempic contains semaglutide, which works through the GLP-1 receptor alone. Mounjaro contains tirzepatide, which targets the GIP receptor as well. Tirzepatide has shown more weight loss in clinical trials comparing the two. Semaglutide has proven cardiovascular benefits, as it reduces the incidence of heart attacks and strokes, while the equivalent trials for tirzepatide had not reported. Which benefit matters more will depend on the reason for taking the medicine.
Is Rybelsus the same as Ozempic?
Both of them contain semaglutide. However, the form of the drugs is different. Ozempic is injected once a week. Rybelsus is a tablet that is taken every single day. The tablet has to be taken on an empty stomach with a small sip of water and nothing else should be consumed for at least thirty minutes, as semaglutide is poorly absorbed from the gut when taken with food and drinks.
