Why interactions matter more with tramadol
Tramadol is a weak opioid, but it also raises serotonin and noradrenaline. That second action produces two risks the other opioid painkillers do not carry, and both become more relevant as the dose climbs.
Seizures. Tramadol lowers the threshold at which a seizure can happen. Seizures have been reported at normal doses of tramadol as well as high doses. The risk is higher for individuals who have experienced seizures in the past, or who are also taking other medications that lower the threshold for seizures to occur.
Serotonin syndrome. The second risk of tramadol is the risk of serotonin syndrome. Serotonin syndrome occurs from too much serotonin in the nervous system. Symptoms include agitation, fast heartbeat, shivering, sweating, muscle twitching and confusion. Serotonin syndrome is a medical emergency.
Antidepressants matter most here. SSRIs, SNRIs, tricyclics and MAOIs all increase the levels of serotonin in the body. Taking one of these medications with tramadol increases the risk of both serotonin syndrome and seizures. Migraine triptans also increase serotonin levels. Let any prescriber know that you are taking tramadol before starting something new.
Other interactions. Other medicines interact with tramadol for a different reason. Paxlovid contains ritonavir, which raises the level of tramadol in the blood. Blood thinners such as warfarin and rivaroxaban (Xarelto) can also interact with tramadol. Alcohol, sleeping tablets and medicines containing benzodiazepines can worsen the drowsiness caused by tramadol and deepen its effect on breathing.

People taking MAOI antidepressants should not take tramadol. People with severe liver or kidney problems, or breathing conditions, may need a lower dose or a different medicine altogether.
How does tramadol 200mg compare with the other strengths?
All of the prolonged-release versions of tramadol work in the same way and differ only in how much they deliver. The 50mg is the exception, being an immediate-release formulation used for pain that comes and goes rather than constant pain. Regardless of strength, the total an adult takes should not exceed 400mg in 24 hours.
