Of the four risks of ecstasy pills that DIMS lists, hyperthermia is the one that can turn an unpleasant night into an emergency. MDMA increases body temperature directly, and it does so while people are dancing in a warm room. That is two further sources of heat arriving alongside the drug. The dose is the third factor.

Why these three matter together

None of these factors can explain a case of hyperthermia alone. Taking a large dose of ecstasy quietly in a cool room is very different from taking a moderate dose while dancing for several hours in a very hot room. This is why the milligram figure never answers the safety question by itself, and why the effects vary between one room and another.

Where 270 milligrams sits

DIMS measured the average Dutch ecstasy pill in 2023 at 138mg of MDMA, with half the market between 117mg and 163mg. A claimed 270mg is close to twice the average content, and close to the level that resulted in a Red Alert in December 2023, when a pill was measured at over 300mg. The EU Drugs Agency has separately reported fatalities following a dose of 300mg of MDMA.

Ecstasy pills represented 58% of everything that DIMS received in 2023, averaging 4.10 euro per pill.

The advantages people report

The effects of ecstasy according to DIMS are a sense of intimacy with others, increased energy and euphoria. The EU Drugs Agency reports heightened sensory awareness and a mild central stimulation. The terms empathogenic and entactogenic were coined to describe the socialising effect of MDMA.

What is hyperthermia?

A body temperature high enough to cause harm. It is not the same as feeling hot. Once the body reaches a certain temperature it is no longer able to regulate itself, and the organ damage that follows is what makes hyperthermia dangerous rather than uncomfortable.

Does a stronger pill mean proportionally more risk?

Not in proportion. A higher dose of MDMA does increase the risk of hyperthermia, but the temperature does not rise in step with the dose. What a higher dose does is reduce the margin, so an environment that would have been tolerable becomes the thing that tips it.

Why does temperature in the room matter so much?

Because the body sheds heat into its surroundings. That only works if the surroundings are cooler than the body. In a hot, crowded room that route is removed, at the same moment the drug and the exertion are both adding heat.
