What is clonazepam used for?
Clonazepam is used mainly for epilepsy. It is effective in treating several types of seizures, including myoclonic and akinetic seizures, and Lennox-Gastaut syndrome. It is also used in panic disorder. This is different from most benzodiazepines, which are prescribed mainly for patients with anxiety.
How it is taken. For seizure disorders an adult usually takes between 2mg and 8mg a day. For panic disorder the maintenance dose is usually between 1mg and 4mg a day. The starting dose is low and then increased over several days rather than starting at the full amount.
The long half-life changes everything. Clonazepam has a half-life of 30 to 40 hours. That is much longer than most benzodiazepines. After a single dose of clonazepam, the drug will still be working on the body more than a day later. With daily dosing, clonazepam reaches a steady level in the body after five to seven days.
That build-up is why the first week feels different from the second. As the level climbs, people may notice drowsiness the following day, slowed thinking, poor coordination and memory problems. In older people the practical consequence is falls, which is why doses are kept low in elderly patients.
Never take it with an opioid. Taking a benzodiazepine together with an opioid painkiller is dangerous. Both classes of drugs slow breathing rates, and combining them creates respiratory depression greater than either drug alone. The FDA added a boxed warning about this danger in 2016, and multiple studies have found that deaths due to opioid overdose increase up to four times over when benzodiazepine drugs are also taken.
Side effects of clonazepam 2mg. The common side effects of clonazepam are drowsiness, unsteadiness, dizziness, and problems with coordination and memory. Clonazepam can impair driving ability, and the effects of the drug can last into the next day. Alcohol will make all of this worse.
Stopping it. Clonazepam must never be stopped abruptly by someone who has been taking the drug regularly. For patients with epilepsy, stopping the medication abruptly can cause seizures that do not stop on their own, which is a medical emergency. The long half-life also delays withdrawal, so symptoms may not appear for a day or two after the last dose and then take longer to settle.
