Why the dose builds up
Because the half-life is around 28 hours, a dose taken tonight has not left the body by tomorrow night. Taken every night, the level climbs over the first several days until it settles. At 10mg that build-up is larger, and the next-day effects tend to increase across the first week rather than fade.
Older patients are affected far more. In one surveillance report, effects on the central nervous system were recorded in 11 percent of patients aged 80 and over. Unsteadiness and the danger of falling are why the starting point for these patients is 2.5mg rather than 5mg or 10mg.
Seven to ten nights. Treatment with nitrazepam should not usually run beyond seven to ten nights of consecutive use. This is shorter than the length of treatment generally given for most benzodiazepines, and the long half-life is the reason. Any treatment beyond that window needs a decision from the prescribing doctor rather than a repeat prescription.
The combination to avoid. The risk that matters most at this dose is combining it with an opioid painkiller. Both benzodiazepines and opioids can slow breathing, and combining the two classes can have an effect much greater than either drug alone. There is a boxed warning for this combination, so it should be avoided altogether.

Not for more than a short stretch. The limit is usually seven to ten nights, and the medicine accumulates in the system across that time rather than clearing each morning. Beyond that point it loses some of its effect as the body adjusts to having it, which is also what makes stopping harder later.
What is the difference between nitrazepam and other sleeping tablets?
Mainly how long they stay in the body. Nitrazepam has a half-life of around 28 hours, making it one of the longest-acting sleeping tablets available. Shorter-acting sleeping tablets clear faster and leave less grogginess in the morning, but they may not hold through the whole night.
How do you stop taking nitrazepam?
