Dependence happens at ordinary doses
Alprazolam dependence has been documented at ordinary doses of the drug, not just high ones. Even within the 0.75mg to 4mg a day range, taken over short periods, there is some risk of developing dependence. The risk increases at doses above 4mg a day, and with treatment lasting beyond twelve weeks.
What the discontinuation studies showed. Studies in which patients stopped taking alprazolam have shown higher rates of rebound and withdrawal symptoms than in patients given a dummy tablet. This discontinuation signal is stronger than that given by most benzodiazepines.
Why coming off is harder than with other benzodiazepines. Due to the short duration of alprazolam's effect, it is more difficult to come off than a longer-acting benzodiazepine. Blood levels of the drug fall between doses, leading to repeated small withdrawals while treatment is still continuing.
Never stop abruptly. Alprazolam should never be stopped suddenly after regular use. Suddenly stopping the drug can lead to severe symptoms of withdrawal, including severe anxiety, insomnia, tremor and even seizures. The drug is discontinued slowly according to a schedule set by the prescribing doctor. Another benzodiazepine with a longer half-life is sometimes introduced first.

Is Xanax 2mg a high dose?
It is a high strength. Alprazolam is one of the most potent benzodiazepines by weight, so 2mg will have more of an effect than 2mg of most other benzodiazepines. For panic disorder the daily total is usually 2mg to 5mg taken in divided doses, so a single 2mg tablet is roughly a whole day's low dose.
How do you come off alprazolam?
The dose is reduced slowly under the direction of a prescriber. Small steps in the reduction are taken with time between each step. Because alprazolam wears off so quickly between doses, the tapering period usually takes longer than for other benzodiazepines. It is normal for anxiety to return during the reduction, which is another reason to reduce the dose slowly.
What happens if you stop alprazolam suddenly?
Severe rebound anxiety, insomnia and tremor are the common results, and seizures are possible after regular use. Studies of withdrawal have shown that alprazolam causes these symptoms at a higher rate than a dummy tablet. That is why the tapering process is planned in advance rather than done suddenly.
How does alprazolam compare with other benzodiazepines?
Alprazolam is the most potent benzodiazepine by weight. Its useful effect lasts the shortest time of any of them, at three to four hours, though it stays in the body for about eleven. That is the reason anxiety returns between doses, which is not true of the benzodiazepines with longer periods of effect. Alprazolam also has active breakdown products and is affected by many other medicines, while lorazepam and oxazepam are cleared more simply.
